Summary: We present the case of an 81-year-old male with metastatic urothelial carcinoma, treated for seven months with pembrolizumab, who presented with a 5-month history of erythematous patches and tense blisters covering approximately 50% of the body. This case underscores the importance of timely recognition and diagnosis of DIBP in patients undergoing ICI therapy, but also highlights the need for individualized treatment approaches and multidisciplinary collaboration when managing adverse cutaneous reactions.
