Summary: A 27yo woman presenting with acute PV was treated with high-dose steroids and rituximab and presented erythematous erosions with scalloping of the borders on the genitalia. HSV culture sensitivities returned, showing acyclovir resistance, expected due to the lack of clinical improvement. In the case of genital lesions that could easily be interpreted as PV lesions, providers should have a low suspicion of acyclovir-resistant HSV and initiate proper treatment immediately.
