Summary: This case poses a diagnostic challenge as the CNS involvement of DCLBL can present initially as isolated third nerve palsy with negative workup, mimicking ischemic cranial neuropathy. Obtaining the correct diagnosis is extremely important as the CNS involvement indicate a different approach of treatment with CNS-directed therapy. Delayed or misdiagnosis can lead to worse clinical outcomes.
