Summary: A case is presented of a 76-year-old man with a history of hydrocele for two years. He went to the emergency room due to constant pain. The ultrasound revealed a solid mass with areas of necrosis. A scrotectomy was performed and a diagnosis of partially necrotic epitheloid mesothelioma was made. A review of the literature and the role of BAP1 in immunohistochemical diagnosis is made.
